Submissions are reviewed manually, anonymised before publication, and used to share lessons across the industry. Your identity is held in confidence and never published.

Before you submit: Do not include named operators, sites, vendors, personnel, or commercially sensitive information. The register publishes lessons learned, not identifying details. If your incident is the subject of an active regulatory investigation, please ensure regulatory reporting has been completed first.
